Trichotillomania is the medical name for compulsive hair pulling. Most clients who come to me for this reason have already done the hard work of getting into therapy, working with a psychiatrist or CBT specialist, and stopping the pulling. What they need from me is a way to see themselves in the mirror again while their hair grows back.
The Rule I Never Break
I never install mesh integration on a client who is still actively pulling. Not because it would not look better, but because the install itself can become a source of pulling behavior. Every trichotillomania client I take needs to be at least 90 days pulling-free and working with a mental health professional. The install supports recovery. It does not replace treatment.
Why Mesh Integration Fits Recovery So Well
The hair from a trichotillomania flare is fragile. It has been pulled, stressed, and needs time. Mesh integration puts a mesh foundation between the client's natural hair and the extensions, so no direct tension goes on the recovering follicles. It also gives the client coverage that stays in place through daily life, which many recovery clients find grounding.
Consultation Considerations
Ask about the client's therapy or treatment status. Ask what they need this install to do for them emotionally. Do not touch the scalp before asking permission. Some clients are still sensitive about their scalp being examined. Photograph gently. Explain everything you are doing before you do it.

Success in this chair is not just full-looking hair. It is a client who can go out of the house, go to work, sit in a meeting, or attend a family event without spending mental energy on their hair. The install gives them back the mental space that was going to concealment strategies.
Working Alongside Mental Health Care
Ask if you can be listed as part of the client's support team, and if the client wants, coordinate maintenance with their therapy schedule so the two work together. Some clients want their therapist to know they are in your chair. Some do not. Respect either.
