Traction alopecia is heartbreaking because it is preventable. It happens when a stylist installs extensions too tight, when a client wears braids too tight for too long, when a beaded row was locked down harder than it should have been, or when a wig was worn under tension. The client sits in my chair afraid of ever touching their hair again. Mesh integration is often the answer.
What Traction Alopecia Looks Like
Traction alopecia usually shows up as hair loss along the hairline, around the temples, at the crown, or wherever the tension was applied. Early traction alopecia is recoverable with rest and no tension. Late-stage traction alopecia can involve permanent follicle damage, especially at the hairline.
Why Mesh Integration Is the Right Rebuild Method
Standard extensions require anchor hair. If the client has just come out of a traction alopecia flare, their existing anchor hair is fragile and cannot take another install that concentrates weight on individual strands. Mesh integration distributes weight across the mesh foundation, which is the opposite of what caused the damage in the first place.
Consultation Rules for These Clients
Do not touch these clients until the active loss has stopped and there is at least 3 months of stable regrowth. Photograph the scalp thoroughly. Explain that the goal of the install is coverage without tension, not maximum volume. Take an honest look at whether any of the loss is permanent, and set client expectations accordingly.
Install Adjustments for Traction Alopecia Clients
Use the lightest weft weight possible. Avoid installs along the exact hairline where damage occurred. Extend the mesh foundation to cover a larger area so weight distribution is broader. Book maintenance every 4 weeks for the first 3 visits so any tension is caught and corrected immediately.
The Client's Emotional Reality
These clients often blame themselves for the damage. Do not reinforce that. Most traction alopecia comes from someone else's mistake. Meet the client with warmth and offer a real path forward.
